REGISTRATION TYPE
Permanent Patients
If you plan to be here for more than three months, you can register as a permanent patient.
- Children
When registering children under the age of five, please bring their immunisation record to be uploaded on to their record to ensure they are up to date with all their vaccinations. - Temporary Patients

Out Of Area Patients
If you live outside of our boundary but wish to register, please contact our staff about becoming an 'Out of Area' patient as we may be able to register you and your family on this scheme. 'Out of area' registration is voluntary for GP practices and can only be considered if it is clinically appropriate and practical in your individual case and is subject to periodic review.
INSTRUCTIONS
1. Complete the New Patient Registration Form.
2. Please complete the New patient questionnaire for children as well as adults (just skip the fields which are not applicable to a child).
Please note that both the new patient registration and questionnaire must be completed and submitted for adults & children before your registration can be processed.
3. Complete the Email and SMS consent form
4. Register for the Online services - this will enable you to view aspects of your medical records after registering via NHS App or Patient Access.
5. When we have received your registration forms & questionnaire, we will contact you to confirm that your registration is being processed. We will then send you a text requesting a photo of a personal identity document (driving licence or passport). Once you send this to us, we will finalise your registration.

- Please note that you will be registering with the practice rather than an individual GP.
NEW PATIENT CHECKS
Please book an appointment to arrange a new patient check if you are:
- Aged 16 and over on regular medication
- Have a significant health problem
- An International patient
